WHAT IS THE LONG-TERM CARE INSURANCE SYSTEM

In the city of Sumida, over 18% of the population is over 65, which makes the timely arrival of this new system ideal, to support our aging society.
This nursing care system began in April 2000 and is a new insurance system designed by the society, as a whole, to care for the aged.
The operation of this system is funded by both the long-term care insurance premiums and the taxes that residents pay.
WHO WILL BE REGISTERED IN THE LONG-TERM CARE INSURANCE SYSTEM

Residents within Sumida who are age 40 or older will be registered in this system following these conditions:
Category 1 - Age 65 or older
Category 2 - Age 40 - 64
There are certain limitations on the services provided depending on the category one is registered in and the insurance premiums being paid.

All eligible individuals are automatically registered within this long-term care insurance with the exception of those 65 or older who must register with 14 days of the following conditions.

Home Care Assistance Centre
Those aged 65 or older who are bedridden or senile and require constant care as well as those aged 40 - 64 who become senile or brain-damaged as a result of aging, may utilize this system.
To use this care service, one must first apply to determine the level of care which will be necessary. Please apply at either the Senior Citizens' Welfare Division or the Home Care Assistance Centre.

After applying for the care, there will be a home visit to evaluate the applicant's daily living conditions. This report along with a statement from their regular doctor are presented at a nursing care authorization board meeting, headed by medical, health and welfare specialists, to determine the extent of nursing care necessary.

Based on the results of the meeting, the maximum services available to be used every month are decided. From here, the nursing care services plan is designed (Care Plan) and the services may begin. Regarding the designing of the plan, please consult with the Care Plan Manager.
FEE FOR SERVICES

Individuals who receive nursing care services under the Long-term Care Insurance System are responsible for covering 10% of the total cost of the designated services. If the services exceed those designated, then the individual must cover for all incidental costs, in addition to the 10% charge.
In the case that this 10% charge becomes costly, one may apply for financial assistance so please make an inquiry.

Category 1 - (Age 65 and older) The fees for this category are divided into five stages depending on one's income. For individuals who receive 180,000yen or more a year in public or corporate pension income, the insurance premiums are automatically deducted from that income. Individuals whose pension income is less than 180,000yen are required to pay by either bank transfer or by direct payment.
Category 2 - The long-term care insurance premium is calculated using the individual's health insurance premium which is based on the income of that individual. This insurance premium is collected together with the health insurance premium.

LONG-TERM CARE INSURANCE SERVICES

Medical advice on bathing, diet and other nursing care to help the recovery of an individual. |
| Short Stay Nursing Care | Short-term admission into a nursing home for the elderly to provide assistance in daily activities. |
| Home Visit Bathing Care | A vehicle equipped with a portable bath is brought to the home to provide bathing assistance. |
| Home Visit Rehabilitation | A physical therapist or other medical practitioner conducts therapy to help the patient maintain or recover bodily functions to achieve self-sufficiency in daily life. |
| Home Visit Nursing Care | A registered nurse or public health nurse visits the home to provide nursing assistance. |
| Home Visit Medical Advice | Treatment-based medical advice by a doctor or dentist who visits the home. |
| Rentals and Reimbursements of Health Care Equipment | Rentals and reimbursements for purchasing health care equipment such as hospital beds and wheelchairs. |
| Reimbursements for Home Health Care Remodeling | Financial Assistance for small-scale home improvements such as the addition of hand rails or the removal of steps. |
| Nursing Care for Senile Patients in a Group Home | Nursing care for the elderly senile residing in a group home. There are five levels of care provided depending on the care required. |

| Insurance-based Nursing Homes for Seniors | Patients will be admitted to these facilities when illnesses have been stabilized and medical care, especially rehabilitation, is required. There are five levels of care provided depending on the care required. |
| Medical Facilities for Nursing Care | Patients will be admitted to facilities with numerous nursing care staff, such as hospitals, to provide relatively long-term medical treatment. There are five levels of care provided depending on the care required. |
Note: The patients are required to cover 10% of the total costs incurred. Should certain other services be offered by both the Welfare Service and the Long-term Care Insurance Service, please utilize the latter service.
